New Patients

How to register

We accept patients onto our list who live within the catchment area of the Practice. To register with the Practice you will be required to complete the appropriate forms (available from reception or by clicking here) We may ask you for evidence of identity, address and the name and address of your previous doctor. If you have an NHS Medical Card. please bring this with you.

You will be asked to make an appointment with the Health Care Assistant for a new patient check.  This completes your registration with the Practice. 

Please note we will note accept a registration for a child unless a parent or guardian is registered with us.

The practice boundary includes the following areas:

South Nash, Maindee, Somerton, Liswerry, Corporation Road, Christchurch, Beechwood, Eveswell, Lysaght Village, East Side River Front, Clarence Place.

If you move out of our practice area we will request that you find a surgery more convenient to your new location
